Ozarks Healthcare to offer free athletic physicals for West Plains students July 21

Surrounding West Plains area clinics to offer free physicals by appointment through remainder of 2022-2023 school year
To help its community prepare for the upcoming school year, Ozarks Healthcare will offer free athletic physicals for student athletes entering grades 7 through 12 in the 2022 – 2023 school year from 4:30 p.m. to 7:30 p.m. on Thursday, July 21, at Ozarks Healthcare Therapies located at 1111 Kentucky Ave. in West Plains. Local physicians, family nurse practitioners, and licensed practical nurses (LPNs) will conduct the physicals. 
Male student athletes will need to check in through the garden entrance near the Shaw Medical Building (where Ozarks Healthcare Therapies is located). Female student athletes will need to check in through the automatic sliding doors located outside the Shaw Medical Building. Once checked in, students will be directed to the appropriate location to complete their athletic physical. 
All students are required to submit a signed consent/medical history form and provide current insurance information from a parent or legal guardian at the time of the physicals. Forms are also available online at https://www.ozarkshealthcare.com/about-us/for-community/. 
Athletic Physicals Offered in Ozarks Healthcare’s Rural Locations
Free athletic physicals will also be offered at all of Ozarks Healthcare’s clinics surrounding West Plains for area students by appointment effective immediately through the remainder of the 2022–2023 school year. To make an appointment for your child’s physical, simply call any of Ozarks Healthcare’s clinics in Alton, Gainesville, Mountain Grove, Mountain View, Thayer, Winona, and Mammoth Spring, Arkansas. Students are required to submit the same signed consent/medical history form and provide current insurance information from a parent or legal guardian as mentioned above at the time of physicals.
