McDonalds #1 West Plains, Violations: 1. Limitation regarding the use of wiping cloths not being met as evidenced by sanitizer strength to low in wiping cloth container. 2. Observed cleaning needed under prep table in coffee/drive thru area. All corrected on site.
KC’s Sports Bistro, Violations: 1. Employees not washing hands properly. Observed hand sink blocked by cart and filled with dirty towels. 2. Hair restraints not being worn by all food preparation staff. 3. Failure to maintain cold potentially hazardous foods at a safe cold holding temperature of 41 degrees or lower. Observed salad bar products at 51 deg and prep cooler at 50 degrees. Add more ice level to salad bar and repair cooler. 4. Temperature measuring device not observed in all coolers/freezers. 5. No paper towels at prep area hand sink. Re-inspection Date: 05/31/2024 
First Baptist Food Pantry, Violations: 1. The storage room walls not smooth and easily cleanable. Observed Walk in freezer/ Coolers room with drywall only. Walls must be smooth and easily cleanable. No reinspection required, will repair as time allows.

Ozark Pizza Company, Violations: 1. Equipment for cold holding does not meet requirements as evidenced by prep cooler at 48 degrees at time of inspection. 2. Establishment’s plumbing in disrepair. Observed leak under sink area in basement in need of repair. No re-inspection needed. 
Mugs, Violations: 1. Observed pizza oven in need of cleaning due to food debris buildup. No re-inspection needed 
Kentzio Japanese Steakhouse, Violations: 1. Observed foods stored uncovered in three-door freezer in need of cover to prevent possible contamination. 2. Failure to maintain cold potentially hazardous foods at a safe cold holding temperature of 41 deg or lower. Observed prep cooler at 52 deg at time of inspection. 3. Nonfood-contact surface(s) of equipment not constructed of a corrosion-resistant, nonabsorbent, and smooth material. Observed wood shelving in need of paint, sealing or replacement. Must be smooth and easily cleanable. 4. Excessive air gap observed with back door propped open. Must keep door closed to prevent entrance of pest and insects. 5. Observed paper towel dispenser in women's restroom not working properly. Paper towels must be dispensed. 6. Observed cleaning needed at grill/fryer floor and dishwasher walls area. Re-inspection Date: 6/01/2024.

Sapz Thai, Violations: 1. Food not protected from contamination. Observed food product boxes stored on floor in back storage area. Keep off ground to prevent possible contamination. 2. Temperature measuring device not observed in all prep coolers. Place thermometers in all coolers for temp monitoring. 3. Observed fryer/grill area flooring and food equipment, utensils in need of cleaning due to food debris buildup. No follow up required. 
Ozark Nutrition and Energy, Violations: 1. Observed mixer stored on cloth towel. Must store on non-absorbent surface to prevent possible contamination. 2. Observed paper towels needed  at employee restroom hand sink. No follow up required. 
Meadowbrook Natural Foods, Violations: 1. Food not protected from contamination. Observed food boxes on floor in walk in freezer. 2. Equipment and items not in use stored inside facility. Observed cleaning/organizing of back area and front area for cleaning purposes. Remove unnecessary items. Will follow-up at next routine inspection.  
Jimmies Family Restaurant, Violations: 1. Observed ash tray in dry storage room being used for smoking. Must only allow smoking outside to prevent contamination. 2. Hair restraints not being worn by all food preparation staff. All food prep employees must have hat, hairnet or visor on. 3. Failure to maintain cold potentially hazardous foods at a safe cold holding temperature of 41 degrees or lower. Observed walk in cooler at 50 degrees. Must move product to other coolers until temp can be maintain below 41 degrees. 4. Observed soda syrup area and ice machine in need of cleaning. 5. Observed no paper towels at back prep hand sink and at employee restroom hand sink. 6. Observed wall area in need of repair in dish area. Must be smooth, easily cleanable and no holes. Re-inspection Date: 6/02/2024

Boys & Girls Club of the Greater West Plains Area, Violations: 1. Temperature measuring device not observed in all prep coolers. 2. Observed prep area hand sink without paper towel dispenser. No reinspection needed.

Subway #2, West Plains, Violations 1. Incorrect order of wash-rinse-sanitize in the three-compartment sink. Observed dishes being washed and then placed directly into sanitizer solution, without proper rinsing. 2. Observed soda fountain in need of cleaning due to debris buildup. Corrected on site.
Taco Bell, Southern Hills, Violations: 1. Ready-to-eat food not discarded as required. Observed bowl/equipment used for fry preparation with label/time stamp to discontinue use at 1:11 PM. Time of inspection at 2:40 PM bowl was still being used. Must follow time stamps and discard use at stated date and time. Corrected onsite.
Snappy East, Violations: 1. Observed soda fountain syrup storage room and walk in cooler in need of cleaning. 2. Observed walk in cooler wall in need of repair due to damage. Must be smooth and easily cleanable. Will correct prior to next routine inspection.  
Senor Valentin Mobile Food/Restaurant, Violations: 1. Failure to properly date label all refrigerated, ready-to-eat potentially hazardous food. All ready-to-eat foods must have a use by date. 2. Temperature measuring device not observed in all prep coolers/freezers. 3. Ware washing sink used to store dirty dishes in all sinks. Must wash as needed and not keep dirty dishes in all sinks. 4. No hot water at prep area hand sinks. Must reach 100 degrees for proper handwashing. 5. Sleeping quarters in an area directly opening into a food establishment. Must remove bed from prep area. 6. Observed dish/prep area walls in need of finishing/repair. Must be smooth and easily cleanable. 7. Unlabeled spray bottle found in prep area. Label all spray bottles for easy identification. Will re-inspect in 2 weeks.

Observed ice scoop not stored properly, laying on top of ice machine unprotected. Store protected to prevent possible contamination. 3. Observed paper towel dispenser not operating properly at prep area hand sink. Corrected onsite

Snappy Mart #49, Violations: 1. Observed countertop under Coke soda fountain in need of cleaning. Corrected onsite. New food prep kitchen will be opening soon. Will call for inspection prior to opening.
Skillets Cafe, Violations: 1. Limitation regarding the use of wiping cloths not being met as evidenced by wiping clothes not stored in sanitizer when not in use. 2. Failure to maintain cold potentially hazardous foods at a safe cold holding temperature of 41 deg or lower. Monitor prep line coolers to ensure temp is maintained at 41 deg or lower. 3. Equipment not maintained in a state of repair or proper adjustment. Observed walk in freezer in need of repair due to ice accumulation. 4. No hot water at prep area hand sink. Must have hot water at least 100 deg for handwashing purposes. 5. Observed prep area flooring in need of cleaning due to food debris buildup. Will re-inspect in 2 months. Food handlers class for entire staff scheduled for May 23.

West Plains Country Club, Violations: 1. Limitation regarding the use of wiping cloths not being met as evidenced by wiping clothes stored on prep areas when not in use. Keep wiping cloths in sanitizer solution when not in use. 2. Temperature measuring device not observed in all prep coolers. Keep thermometers available for temp monitoring. 3. The nonfood-contact surface of the walk-in freezer shelving not designed and constructed for easy cleaning and maintenance. Observed shelving with rust. Must be smooth and easily cleanable. No follow-up required.
West Plains Civic Center, Violations: 1. Temperature measuring device not observed in upstairs refrigerator for proper temp monitoring. 2. Equipment not maintained in a state of repair or proper adjustment. Observed deep freeze in need of defrosting due to ice accumulation. No follow up required.
Snider’s Family Restaurant, Violations: Hair restraints not being worn by all food preparation staff.  All prep employees must have hair restraint (hat, hairnet, or visor). 2. Improper thawing of Potentially Hazardous Foods. Observed frozen product thawing at room temperature. Must thaw in refrigerator, cool running water or microwave. 3. Observed outside dumpster not located on a non-absorbent surface. Place on concrete pad. 4. Absence of handwashing signage at all sinks used by employees. Must place handwash signage at employee restroom sink. No reinspection required 
Smart Mart LLC, Violations: 1. Observed floor repair needed at service area of store. Floor must be smooth and easily cleanable. Corrected onsite.
JJ’s Not Your Normal, Violations: 1. Failure to maintain hot potentially hazardous foods at a safe hot holding temperature of 135 degrees or higher. Observed warmer with corn and other product at 118 deg and pulled porker in warmer dish at 125 degrees. 2. Observed clean items stored on cloth towel. Store on non-absorbent surface to prevent possible contamination. 3. Observed several flies throughout prep area. Keep doors closed and place fly traps in areas not over food prep. Will re-inspect in 3 months.

West Plains High School Cafeteria, Violations: 1. Limitation regarding the use of wiping cloths not being met as evidenced by wiping cloth sanitizer bucket concentration to low. 2. Failure to maintain cold potentially hazardous foods at a safe cold holding temperature of 41 degrees or lower. Observed salad bar at 48 degrees with only 1 strip on ice packs below food. Must use additional ice to keep product at required temp. No follow-up required.
West Plains Elementary School Cafeteria, Violations: Limitation regarding the use of wiping cloths not being met as evidenced by wiping cloth sanitizer not being used or not high enough concentration. Re-inspection Date: 6/09/2024
