West Plains Quickstop #1, Pomona, violations: 1- Hair restraints not being worn by all food preparation staff. Observed hair restraint not being worn by food prep staff. 2- Failure to properly date label all refrigerated, ready-to-eat potentially hazardous food. Food that had been prepared, stored in WIC was not properly dated or labeled for proper identification.  3- Unsafe food not discarded or reconditioned according to an approved procedure.  observed cheese being stored for deli use with mold. 4- No hot water at prep area handsink. Observed no hot water available at prep area handsink, only cold water faucet worked. Follow up date February 8.

Dollar General Market, West Plains, violations: 1- Food not protected from contamination. Observed boxes on the floor in the walk-in cooler and walk-in Freezer. They will move boxes.

Town & Country Supermarket, Willow Springs, violations: 1- Food not protected from contamination. Observed food uncovered being stored in deli walk-in cooler. 2- Unsafe food not discarded or reconditioned according to an approved procedure. Observed chicken drumsticks, smoked pork jowls, and pork back ribs past the sell by date. 3- Equipment not maintained in a state of repair or proper adjustment. Observed ice buildup on walk-in freezer floor and ceiling. 4- Observed debris accumulating on tracks of the deli display cases, in need of cleaning. 5- Observed on outer back wall of meat department in storage area missing part of the wall. The inner part of the wall appeared to have mold and not in stable condition. Reinspection date February 14.

Los Amigos Restaurant, Mountain View, violations: 1- Bulk items not properly labeled. Observed bulk items not properly labeled and outside of their original packaging. 

Jason's Mexican Restaurant, Willow Springs, violations: none.

January 23

San Juaneros, West Plains, on site due to complaint, violations: 1- Food not protected from contamination. Observed food uncovered in bottom of prep table cooler. Also observed bulk item containers with lids open in storage area, with doors open. 2- The grease trap is not accessible for cleaning. Observed evidence of grease being dumped outside, not utilizing the grease trap. Also observed evidence of the mop sink not being utilized. Reinspection date February 22.

McDonald's #1, West Plains, violations: 1- No hot water at two prep area handsinks. Observed two prep area handsinks not reach 100 degrees after one minute of running. Handsinks must reach 100 degrees in one minute for proper handwashing. Reinspection date February 22.
