McAlister's Deli, West Plains, violations: 1- Failure to maintain cold potentially hazardous foods at a safe cold holding temperature of 41 degrees. Observed prep cooler above 41 degrees after multiple temp checks. All cold held food items must be held at or below 41 degrees. Re-inspection Date August 16.
Ozarks Healthcare Foundation Coffee Shop, West Plains, violations: none.
Ozarks Healthcare - Grill 59, West Plains, violations: 1- Food not protected from contamination. Observed boxes on the floor in walk-in and dry storage area. Corrected on site. 
August 14
Fry Time, Mansfield, mobile, violations: none. Approved for opening/operation in Howell County.
August 18
MO Sno Co, West Plains, mobile, violations: none.
August 19
The Pasta Place, Willow Springs, violations: none. Discussed proper employee hygiene practices based on questions from new employees.
The Gathering Place Drink Company, Willow Springs, violations: none.
Taste of India, Mountain View, violations: 1- Food not protected from contamination. Observed food being stored in multiple coolers without covers. Food being stored must be covered to prevent contamination. 2- Bulk ingredients not labeled. Observed 1 bulk item not labeled or in original packaging. Also, observed food being stored in original metal container. Once opened, food must be moved from metal container to food safe container to prevent contamination. Corrected on site. 
August 21
Little Caesars Pizza, West Plains, violations: 1- Floor to wall junctions must be covered and impenetrable, floor drains shall be in place and draining appropriately. Observed wall directly behind the 3-vat sink, between 3 vat and pizza ovens, in need of repair. Wall is left open. 2- Observed no paper towels available at either hand sink in the prep area. Paper towels must be available to prevent from drying hands in unsanitary ways. Reinspection date September 20.
The Kid Community, West Plains, pre-opening, violations: 1- Observed some areas in the food prep station not smooth and easily cleanable. Areas especially in the food prep area must be smooth and easily cleanable. Approved to open once bare wood has been painted/sealed and food handlers permits have been obtained.  Discussed proper air-drying procedures and proper labeling/dating of food items.
Craft Café, West Plains, violations: 1- Observed cold held food items in one freezer and the prep table not properly labeled. Cold held food items must be labeled and dated for proper identification and discarding procedures. Reinspection date September 20. 
August 24
The Truck Patch, West Plains, violations: none.
August 26
Kentucky Fried Chicken, West Plains, violations: Observed tiles missing near the walk-in freezer, allowing water to puddle and not make it to the drain. Also observed the door of one walk-in cooler not closing tightly. 2- Observed cleaning needed under the drive-thru soda machine and the back corner wall of the 3-vat sink. Reinspection date September 25.
August 27
Pizza Inn, West Plains, violations: 1- Food not protected from contamination. Observed food uncovered in walk-in cooler. 2- Observed bulk dough containers in need of labels. Also, observed labels needed for food items in the salad bar. 3- Equipment not maintained in a state of repair or proper adjustment. Observed multiple cups with chips and cracks. Equipment and utensils must be in good condition for use. 4- Observed debris buildup in the walk-in cooler and on the ceiling tile near the pizza oven.  One tile is in need of replacement. Reinspection date September 26.
August 28
Wendy’s, West Plains, violations: 1- Observed the faucet of the hand sink leaking. Will repair. 
Sugar Lily Bakery and Floral, West Plains, violations: none.
Anna's Bananas, West Plains, temporary, violations: none.
August 31
Sonic, West Plains, violations: 1- Observed ceiling tiles missing due to leak. Ceiling tiles must be in place to prevent food adulteration and pest entry. Work order placed for repair.
