Java Therapy - Beyond The Bean, West Plains, mobile, violations: 1- Hand-sink under repair. Proper substitute measures met. 

West Plains Motel, West Plains, violations: 1- Pests not appropriately controlled or minimized. Observed a large amount of ants in the corners of the bathroom and corner of living space.  Also, observed pests, (roaches) behind the mini refrigerator. Follow up date June 19.

the Sweet Shop, West Plains, temporary, violations: none.

Taco Bell, Southern Hills, West Plains. On-site due to complaint. Completed routine inspection while on-site. Violations: 1- Observed soda nozzles in drive thru in need of cleaning. 2- Excessive air gap at back door. Observed gap at back door. Reinspection date July 2.

Primo's Grill, West Plains, on-site for follow up from previous follow up & routine inspection. Also, for concern related to the grease trap. Violations: 1- Failure to maintain cold potentially hazardous foods at a safe cold holding temperature of 41 degrees. Observed the prep tables above 41 degrees. All cold held food items must be held at or below 41 degrees. 2- Observed evidence of mice in the facility. Evidence is in the bottom of the cabinet area near the soda machine and the chip station. Also observed evidence in the dish drying area. Temperature guidelines need to be met on the prep tables or food must be moved.  Professional pest control is recommended with receipt for visit. Will discuss the process for the grease trap if repair or replacement is needed.  Follow up date: June 10.

Comfort Inn, Willow Springs, violations: 1- Electrical wiring or wiring components do not meet minimum requirements as evidenced by being unable to test GFCI outlet in room 222. Blockage/issue with outlet would not allow testing device to be inserted. 2- Requirements for relief valve discharge pipes not met as evidenced by one pipe being PVC. Relief valve piping must be galvanized copper. Follow up date June 29.

Quality Inn, West Plains, violations: none.

June 10

Primo's Grill, West Plains. On-site for follow-up. Violations: 1- Failure to maintain cold potentially hazardous foods at a safe cold holding temperature of 41 degrees. Observed cold holding stations above 41 degrees, cold held food items must be held at or below 41 degrees.  2- Observed, based on evidence of drainage pipes, no grease trap is installed or working properly. Grease trap will need to be added for proper dish washing. Pest control seems to be under control.  Will follow-up on temperature control issue in one week, parts ordered for repair. If not solved by next inspection, work order will be issued followed by closure procedures.  Grease trap will need to be installed as well by next routine inspection. Follow up June 17.

Wendy’s, West Plains, violations: none. On-site due to complaint. Complaint unfounded. Discussed proper procedures.

June 15

Super 8, West Plains, violations: 1- Observed the carpet ripped in rooms 110 & 226. 2- Observed various forms of trash in rooms 219, 210, & 207, under the bed and under the cushion of chairs. 3- Observed CO2 detector needed in laundry/electrical room. Detector must be hardwired. 4- No working smoke detector located in laundry room. Follow up date July 9.

Dairy Queen, West Plains, violations: 1- Excessive dust/debris buildup on the vents of the fans in the walk-in cooler. Follow up date July 16.

10 Box, West Plains, violations: 1- No hot water at produce station handsink. 2- Excessive dust/debris and ice buildup in the small meat walk-in freezer. Reinspection date July 17.

Mio Bistro, Mountain View, violations: 1- Improper storage of ice scoop. Observed ice scoop stored in the ice. Ice scoops must be stored in a container outside of ice storage or hung up to where it does not sit in the ice. 2- Observed cold held and bulk food items not properly labeled. Cold held food items were not properly labeled or dated for easy identification or discarding.  Bulk items not labeled for easy identification.

Baymont Inn and Suites, West Plains, violations: 1- Observed A/C vents on the North and South side 2nd floor hallway in need of cleaning.  Observed wall repair needed in 1st floor storage room. 2- Electrical wiring or wiring components do not meet minimum requirements as evidenced by no power at the GFCI outlet in Room 134. 3- Required exit signs not provided as evidenced by multiple exit signs not working properly. Exit signs in the pool hallway, 2nd floor hallway and 1st floor hallway were not working properly when tested. 4- Facility's smoke detectors and heat sensing devices not hardwired with battery backup. Observed smoke detector in 2nd floor storage room not hardwired with battery backup. Follow up date July 17.

Mason's Midwest Meats, West Plains, violations: 1- Food not protected from contamination. Observed boxes being stored on floor of walk-in cooler. 2- Observed ceiling tiles missing in hallway leading to walk-in. Observed small gap at the back door that will need attention as well, possible pest entry. Reinspection date July 26.
